In order to ensure that your first visit is as simple as possible, it will be important for you to bring with you both your insurance card and a photo ID.  We are a specialist's office, which usually carries a slightly higher co-payment. You may also wish to contact your insurance provider in advance of this visit to verify your benefits so that there is no confusion at the time you check in.  As there are several forms that you will need to fill out at the time of your initial visit please try to arrive approximately 20 minutes in advance of your appointment time to complete that paperwork so that we can remain on schedule.

We recognize that now may be a difficult time to talk about finances, but we need to establish an account for you in our billing system.  If you plan to utilize your insurance, your coverage requires strict protocols to justify medical necessity and payment for services.  By partnering together we can do our part to reduce treatment confusion, meet the needs of your insurance carrier, and avoid unnecessary expense for you.
